USERRA Poster required by Department of Labor (DOL) under the Uniformed Services Employment and Reemployment Rights Act as of January 16, 2006. 

FMLA Form WH-380 -The Certification of Health Care Provider is an optional form DOL provides for an employer to use if it chooses to require certification from a health care provider that a serious health condition requiring leave under the Family and Medical Leave Act (FMLA) exists.

FMLA Form WH-381 - The Employer Response to Employee Request for Family and Medical Leave is an optional form the DOL provides for an employer to satisfy its obligation to provide written notification detailing the employer’s specific expectations and obligations of an employee who is taking FMLA protected leave.

FLSA Classification Questionnaire - Unless a position is exempt from the Fair Labor Standards Act (FLSA), the employee who holds the position will be entitled to receive overtime pay if she works in excess of 40 hours per week. One of the most common exemptions from the FLSA is for "administrators" but it is not always easy to tell if the position in question fits the definition. Use this questionnaire to document the employee's and the supervisor's input, and to help you decide.

Background check authorization form - Do not use this form for anything other than obtaining a background check for the purpose of evaluating  a job applicant. The use of background checks is governed by the Fair Credit Reporting Act. Always attach a copy of the Federal Trade Commission's "Summary of Rights Your Rights Under the Fair Credit Reporting Act"  when using this form.
